Student Creator Program

Parent and guardian consent.

This form must be completed by the parent or legal guardian of an applicant age 14–17. It does not collect tax, banking, or identity documents.

Before you sign

What you are approving.

The student may receive a personal affiliate link and earn commission on eligible, approved orders.
The program is free. There is no required purchase, inventory, recruiting, or guaranteed income.
The student must clearly disclose the affiliate relationship, make honest recommendations, and follow school and social-platform rules.
The student should participate only outside school hours and within any work-hour restrictions that apply.
The adult guardian will be the affiliate account owner and payment recipient until the student turns 18 and completes new onboarding.
For a 14- or 15-year-old, Precision 3D may require a school employment certificate or other authorization before final approval.
